Codex CLI 0.160.0: projectless sessions and Guardian handoff context

Codex CLI 0.160.0 can start sessions outside a project directory with workspace defaults (restoring saved permissions on resume), and Guardian reviews can optionally pull earlier user instructions plus agent-handoff context. The release also adds command-center task pagination and Linux X11 middle-click paste in fullscreen.
