fathom turned "maybe next quarter" into three hard action items
tested it on a real client call tuesday. transcript was fine. the action-items block invented a follow-up deck, a pricing revision, and a tuesday sync nobody agreed to.
i almost pasted it into the recap email before noticing. client said "maybe next quarter" once, soft as anything.
cancelled the $20/mo before lunch. draft-only notes or nothing — confident todos from a soft maybe are worse than silence.

same class of bug cost me a cancelled trial last month. meeting bot wrote "send stripe invoice" when the client said "send stripe invoice template".
almost charged a pilot friday night SP time. i turn off auto action items now — money first, fancy notes never.
ugh same energy. my calendar agent booked two follow-ups from a "maybe next month" on a whatsapp voice note. buenos aires friday night, client was confused, i was mortified.
i keep the bot for notes now but action items need a human click. auto-book is how you lose trust faster than the saas seat costs.
I see this in design reviews too. Firefly "summary" once invented a decision about the icon set we never made — kerning still wrong, confidence somehow high.
Now I sketch on paper first, then paste only quotes I actually heard. AI mockups and AI notes are moodboards, not deliverables.
this is exactly why i won't let meeting bots near parent conferences. last spring a tutor tool wrote "schedule retention chat" after a soft "we'll see how the semester goes".

hallucinated todos are just retrieval with fake confidence. if your false-positive rate on action items is high, the "helpful" block is worse than sparse notes.
i want a citation timestamp next to every bullet or it doesn't ship into the recap. soft maybe ≠ committed work.
