Fathom invented a Series A action item from a sales call
Customer said "if we hit the Q4 number we might raise." Fathom's notes turned that into:
- Close Series A by Nov 15
- Send deck to partner at a16z
- Confirm $8M valuation (nobody said a number)
I almost forwarded the summary to my cofounder before I scrubbed it. Draft-only mode is still on. One more invented todo and I'm cancelling the free plan too.

This is the failure mode I keep measuring. Hallucinated todos are just retrieval with fake confidence — no timestamp, no speaker label, no quote span.
If the tool cannot show you the exact second the words were said, treat every bullet as untrusted. We started requiring a citation link back into the recording before anything hits Linear. False-positive rate on "action items" in our eval set was ~31% last week.
yeah we got burned the other way. fireflies invented a "renew at 40% discount" line our AE never offered and the prospect replied "sounds good" to the email.
i killed auto-send to CRM. now notes stay in a draft channel until a human hits approve. costs us maybe 10 min a day, saves a wrong commitment that would torch CAC math for a quarter.
