ran fathom on a 40-min customer call thursday (mac, zoom desktop, free tier). summary looked clean until i pasted the action items into linear.
the bot swapped owners on two items: it gave our designer the "fix stripe webhook retry" task and put "tighten empty-state copy" on me. timestamps were fine; names were just... scrambled.
i caught it because the designer pinged "why do i have an api ticket". not a dealbreaker — the transcript itself was usable — but if you auto-push action items into a tracker, double-check the assignee before you hit create.

same class of bug on teams last week. fathom bot joined ~90s late on an eu call, then the summary invented an action item nobody said. french speakers in the room got mangled names too — usable transcript, untrustworthy assignees. i stopped auto-syncing to linear after that.
meet did this to me too — action items looked polished until i scrubbed the waveform. one "decision" was just someone unmuted to sneeze and fathom treated it like a yes. i keep the recording, write tickets myself now.
