Fathom invented three action items I never said
Had a Zoom with a client Tuesday. Fathom emailed the notes 12 minutes later with three "action items":
- Schedule Series A intro with their counsel
- Rewrite homepage hero by Friday
- Cancel Otter trial
None of those words left anyone's mouth. We talked about a case study outline and whether CapCut was worth $20/mo for ads.
I almost forwarded the email before catching it. Cancelled the seat this morning. Invented todos are worse than an empty notes doc.

yeah this is why we treat meeting notes as untrusted input now. last month Fireflies put a wrong CAC number in the Slack summary and a junior ran with it in a deck. if attribution breaks the tool is just debt. we still record, we just don't auto-push to Notion anymore.
this is the exact failure mode that freaks me out in tutoring tools. made-up homework todos look helpful until a kid does the wrong worksheet. we treat any auto action item as draft-only now — if it can't point at a timestamp in the recording, it doesn't leave the draft.
same energy as Descript Overdub mangling a guest name three times in one episode. "almost forwarded" is the scary part. i leave the ums now. silence beats a phantom Series A.
hallucinated action items are just retrieval with fake confidence. if you cannot cite the timestamp in the transcript, do not write the bullet. we started scoring note tools on false-positive rate, not "coverage". Fathom would fail that eval hard.
I forwarded a Fireflies summary once that invented a "close the Series B" bullet. Client Slack went nuclear for like 40 minutes. Cancelled that seat same afternoon. Empty notes beat a confident wrong Series A every time.
